Responsible for supporting communities as part of Community Health Advocate (CHA) team. CHA's are public health workers and trusted members of the community. They serve as a link between patients and social service agencies in the community to help members gain access to available health programs. Provide consumers with information and assistance to access the health care and enroll our community members in public insurance programs. Utilize knowledge and skills in community health improvement and care management to support the Community Health Needs Assessment Implementation plan.

This position is based to cover our needs in SLC. It is a remote role but the individual needs to live i Utah. We do prefer individuals who are fluent in Spanish and have experience in EPIC. We do require the CHW certification.
