Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients, linkage to resources in the community.

Qualifications

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

 

  • Master Degree Social Work (Required) 
  • Licensed Clinical Social Worker (LCSW) preferred.
  • Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart
  • Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED. Current Driver’s license
  • One year of Social Work required

 

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly

 

This part time hospice role will cover Hospice of Mercy, Durango, Pagosa and the surrounding areas.  This role does offer full benefits. We are looking for someone who can cover weekdays.  There are no weekend requriements for this role.

$30.70 - $49.16 / hour
