Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community.
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
Master of Social Work (MSW)
6 months home care/hospice experience preferred.
Two years post Master's experience in health care or related field preferred.
Licensed Clinical Social Worker (LCSW) preferred.
Current Driver's license Cardiopulmonary Resuscitation (CPR) required
Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
This field hospice PRN Social Worker will work variable days Monday through Friday. The territory for this role is in Frisco, CO.
