This position supports care management and coordination activities in an outpatient setting. Partners with healthcare team to maximize patient engagement and patient outcomes. Identifies patients overdue for visits, labs, or referrals and initiates patient supportive services to reduce negative impact on patients' health and promote efficacy of clinic resources. 

 

This position is remote.

 

Schedule:  Monday - Friday, eight hour days, flexible

Qualifications

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

 

  • High School Diploma/G.E.D.
  • Minimum 3 years of experience in an outpatient medical practice
  • Proficient computer skills – data entry, retrieval, and report generation, excel and virtual meetings
  • Experience with Epic preferred 
  • Certification as a Certified Medical Assistant or Certified Clinical Medical Assistant Required, or LPN Required Diploma from an accredited medical assistant's program and externship completed (if applicable) 

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly

Pay Range

$21.99 - $37.32 /hour
